Aging Life Care Managers have extensive knowledge of the medical, psychological, legal, financial, cognitive, social, and physical challenges of aging. Their expertise allows them to identify and access available resources that help older Americans continue leading lives of purpose and dignity. Care managers also help families navigate complex systems and identify resources while providing support and advocacy. At Brandywine Elder Care Management, we:
We meet with clients in their home or senior living community to evaluate their physical, mental, social and financial health and determine their needs.
We develop a Plan of Care that includes a list of resources to help clients meet the challenges of aging.
Manage and Monitor
We help manage and monitor the care plan to ensure stated goals are being met.
We go with clients to medical and other professional appointments and advocate on their behalf.
We coordinate in-home care services, moving schedules and respite care.
If a crisis occurs, we’ve available to intervene and provide assistance as well as solve problems as they arise.
